Adds support for Kayako 4.68 and also adds new features!

New Feature: Now supports Smart Variables.

Fullname is: {{fullname}}
forename is: {{forename}}
surname is: {{surname}}
title is: {{title}}
salutation is: {{salutation}}
Organizaton is: {{organization}}
Customfield is: {{custom_field[CustomFieldName]}}

When making macros, replies, forwards, or KB inserts, using {{fullname}} for example, will be replaced on submission with the users full name.

Also now supports Kayako 4.68
